Neo and Bane lie unconscious in the medical bay of the ship Hammer. Inside the Matrix, Neo is trapped in a subway station named Mobil Ave (an anagram for limbo), a transition zone between the Matrix and the Machine City. He meets a "family" of programs, including a girl named Sati. The "father" tells Neo the subway is controlled by the Trainman, a program loyal to the Merovingian. When Neo tries to board a train with the family, the Trainman refuses and overpowers him.

Seraph contacts Morpheus and Trinity on behalf of the Oracle, who informs them of Neo's confinement. Seraph, Morpheus and Trinity enter Club Hel, where they confront the Merovingian and force him to release Neo. Troubled by visions of the Machine City, Neo visits the Oracle, who reveals that Smith intends to destroy both the Matrix and the real world. She states that "everything that has a beginning has an end," and that the war will conclude. After Neo leaves, a large group of Smiths assimilates Sati and Seraph. The Oracle accepts her assimilation and the Smiths gain her powers of precognition, but it is not clear if she has actually been lost forever in the process.

In the real world, the crews of the Nebuchadnezzar and the Hammer find and reactivate Niobe's ship, the Logos. They interrogate Bane, who says that he has no recollection of the earlier massacre. As the captains plan their defense of Zion, Neo requests a ship to travel to the Machine City. Motivated by her encounter with the Oracle, Niobe offers him the Logos. Neo departs, accompanied by Trinity. Bane, who has stowed away on the Logos, takes Trinity hostage. Neo realizes that Bane has been assimilated by Smith and a fight ensues. Bane burns Neo's eyes with a power cable, permanently blinding him. Neo, discovering an ability to perceive anything part of the machine source code in the real world as wreathed in golden light, sees and kills Bane. Trinity pilots them to the Machine City.

Niobe and Morpheus rush toward Zion in the Hammer to aid the human defenses. Zion's shipyard is overwhelmed by a horde of Sentinels, and the fatally wounded Captain Mifune instructs Kid to open the gate for the Hammer. When it arrives, it discharges its EMP, disabling all the Sentinels present but also Zion's remaining defenses. The humans are forced to retreat and wait for the next attack, thinking it will be their last stand.

Near the Machine City, the Logos is bombarded by thousands of missiles, causing it to crash, fatally wounding Trinity. Neo enters the Machine City and encounters "Deus Ex Machina," the machine leader. Neo warns that Smith plans to conquer both the Matrix and the real world and offers to stop Smith in exchange for peace with Zion. The machine leader agrees, and the Sentinels stop attacking Zion.

The Machines provide a connection for Neo to enter the Matrix. Inside, the Smith with the Oracle's powers steps forth, saying that he has foreseen his victory against Neo. After a protracted battle, Neo realizes there is no other way to win and allows himself to be assimilated. Now connected to the Source, the machine leader sends the deletion signal to the Smith program through Neo's body in the real world. This causes the Neo-Smith clone and all other Smith clones in the Matrix to be destroyed, deleting Smith once and for all. The Sentinels withdraw from Zion, Morpheus and Niobe embrace, and Neo sees a final vision of the machine city while succumbing to his injuries, as his body is carried away by the machines.

The Matrix is rebooted, and the Architect encounters the Oracle in a park. They agree that the peace will last "as long as it can" and that those humans who desire it will be offered the opportunity to leave the Matrix. When questioned about Neo's fate, the Oracle tells Sati that she thinks they will see Neo again as Sati reveals she created a beautiful sunrise over the horizon in Neo's honor. Seraph asks the Oracle if she knew this would happen. She replies that she did not know, but she believed.

Six months after the events of The Matrix, Neo and Trinity are now romantically involved. Morpheus receives a message from Captain Niobe of the Logos calling an emergency meeting of all ships of Zion.

 Zion has confirmed the last transmission of the Osiris: an army of Sentinels is tunneling towards Zion and will reach it within 72 hours. Commander Lock orders all ships to return to Zion to prepare for the onslaught, but Morpheus asks one ship to remain to contact the Oracle. As the Caduceus receives a message from the Oracle, one of the Caduceus crew, Bane, encounters Smith, who reveals that his previous encounter with Neo severed his connection with the Matrix and has made him a rogue program, then absorbs his avatar. Smith then uses the phone line to leave the Matrix and gain control of Bane's real body.

In Zion, Morpheus announces the news of the advancing machines to the people. In the Matrix, Neo meets the Oracle's bodyguard Seraph, who leads him to her. After realizing that the Oracle is part of the Matrix, Neo asks how he can trust her; she replies that this is his decision. The Oracle instructs Neo to reach the Source of the Matrix with the help of the Keymaker. As the Oracle departs, Smith appears, telling Neo that after being defeated, he refused to be deleted and is now a rogue program. He demonstrates his ability to clone himself using other inhabitants of the Matrix, including other Agents, as hosts. He then tries to absorb Neo but fails, prompting a battle between Smith's clones and Neo. Neo manages to defend himself, but is forced to retreat from the increasingly overwhelming numbers.

Neo, Morpheus, and Trinity visit the Merovingian, who is imprisoning the Keymaker. The Merovingian, a rogue Matrix program with his own agenda, refuses to let him go. His wife Persephone, seeking revenge on her husband for his infidelity, leads the trio to the Keymaker. Morpheus, Trinity, and the Keymaker flee while Neo holds off the Merovingian's henchmen. Morpheus and Trinity try to escape with the Keymaker, pursued by several Agents and the Merovingian's chief henchmen, the Twins. After a long chase, Trinity escapes, Morpheus defeats the Twins, and Neo saves Morpheus and the Keymaker from Agent Johnson.

The crews of the Nebuchadnezzar, Vigilant, and Logos help the Keymaker and Neo reach the Source. The Logos crew must destroy a power plant and the Vigilant crew must disable a back-up power station, to prevent a security system from being triggered, allowing Neo to open the door to the Source. Haunted by a vision of Trinity's death, Neo asks her to remain on the Nebuchadnezzar.

The Logos succeeds, while the Vigilant is destroyed by a Sentinel, with everyone aboard perishing. Trinity replaces the Vigilant crew and completes their mission. However, Agent Thompson corners her and they fight. As Neo, Morpheus, and the Keymaker try to reach the Source, the Smiths ambush them. The Keymaker unlocks the door to the Source, but the Smiths shoot him dead as he closes the door.

Neo meets a program called the Architect, the creator of the Matrix, who explains that Neo is himself integrated with a program and an intentional part of the Matrix, which is now in its sixth iteration. Neo is meant to stop the Matrix's fatal system crash that naturally recurs due to the concept of human choice within it. As with the five previous Ones, Neo has a choice: either return to the Source to reboot the Matrix and pick survivors to repopulate the soon-to-be-destroyed Zion, as his predecessors all did, or cause the Matrix to crash and kill everyone connected to it, which would mean humanity's extinction when Zion is destroyed. Neo learns of Trinity's situation and chooses to save her instead of returning to the Source, to which the Architect responds dismissively.

Trinity is shot as she and Agent Thompson fall off a building. Before she hits the ground, Neo catches her. He then removes the bullet from her heart and revives her. They return to the real world, where Sentinels attack them. The Nebuchadnezzar is destroyed, but the crew escape. Neo displays a new ability to disable the machines with his thoughts, but falls into a coma from the effort. The crew are picked up by another ship, the Hammer. Its captain, Roland, reveals the other ships were wiped out by the machines after someone prematurely activated an EMP, and that only one survivor, the Smith-possessed Bane, was found.

At an abandoned hotel within a major city, a police squad corners a woman (later revealed as Trinity), who overpowers them with superhuman abilities. She flees, pursued by the police and a group of mysterious suited Agents capable of similar superhuman feats. She answers a ringing public telephone and vanishes just before the Agents crash a truck into the booth.

Computer programmer Thomas Anderson, known by his hacking alias "Neo", feels something is wrong with the world and is puzzled by repeated online encounters with the phrase "the Matrix". Trinity contacts him and tells him a man named Morpheus has the answers he seeks. A team of Agents and police, led by Agent Smith, arrives at Neo's workplace searching for him. Though Morpheus attempts to guide Neo to safety via telephone, Neo is captured and coerced into helping the Agents locate Morpheus, whom they regard as a "known terrorist". Undeterred, Neo later meets Morpheus, who offers him a choice between two pills; red to reveal the truth about the Matrix, and blue to return him to his former life. After Neo swallows the red pill, his reality disintegrates, and he awakens naked in a liquid-filled pod, among countless other humans attached to an elaborate electrical system. He is retrieved and brought aboard Morpheus's flying ship, the Nebuchadnezzar.

As Neo recuperates from a lifetime of physical inactivity in the pod, Morpheus explains the truth. In the early 21st century, there was a war between humans and intelligent machines. When humans blocked the machines' access to solar energy, the machines harvested the humans' bioelectric power, while their minds are kept pacified in the Matrix, a shared simulated reality modeled after the world as it existed at the end of the 20th century. The machines have conquered the world, and the underground city of Zion is the last refuge of free humans. Morpheus and his crew are a group of rebels who hack into the Matrix to "unplug" enslaved humans and recruit them; because they understand the Matrix's simulated nature, they can bend its physical laws. Morpheus warns Neo that death within the Matrix kills the physical body, and the Agents he met are powerful sentient computer programs that eliminate threats to the system, while machines called Sentinels destroy rebels in the real world. Neo's prowess during virtual combat training cements Morpheus's belief that Neo is "the One", an especially powerful human prophesied to free humanity and end the war.

The group enters the Matrix to visit the Oracle, the prophet who predicted that the One would emerge. She suggests to Neo that he is not the One and warns that he will have to choose between Morpheus's life and his own. Before they can leave the Matrix, Agents and tactical police ambush the group, tipped off by Cypher, a disgruntled crew member who has betrayed Morpheus in exchange for a comfortable life in the Matrix. Morpheus allows himself to be captured so the rest of the crew can escape. Cypher exits the Matrix first and murders several crew members as they lie defenseless in the real world. Before he can kill Neo and Trinity, Cypher is killed by Tank, a crewman whom he had only wounded.

In the Matrix, the Agents interrogate Morpheus to learn his access codes to the mainframe computer in Zion. Tank proposes killing Morpheus to prevent this, but Neo resolves to return to the Matrix to rescue Morpheus, as the Oracle prophesied; Trinity insists she accompany him. While rescuing Morpheus, Neo gains confidence in his abilities, performing feats comparable to the Agents'. Morpheus and Trinity exit the Matrix, but Smith ambushes and kills Neo before he can leave. As a group of Sentinels attack the Nebuchadnezzar, Trinity whispers to Neo that he cannot be dead, because she loves him and the Oracle told her she would fall in love with the One. She kisses Neo and he revives with newfound power to perceive and control the Matrix. He effortlessly defeats Smith, and leaves the Matrix just as the ship's electromagnetic pulse disables the Sentinels.

Later, Neo makes a telephone call inside the Matrix, promising the machines that he will show their prisoners "a world where anything is possible". He hangs up and flies into the sky.

In 1944, two World War II fighter pilots, American pilot Hank Marlow and Japanese pilot Gunpei Ikari, parachute onto an island in the South Pacific after a dogfight and engage in close combat until the fight is interrupted by a giant ape.

 In 1973, Bill Randa, head of the U.S. government organization Monarch, plans a search for primeval creatures on the recently discovered Skull Island. He recruits a U.S. Army unit commanded by Lieutenant Colonel Preston Packard, tracker and former British Special Air Service Captain James Conrad, and anti-war photographer Mason Weaver. Arriving at Skull Island, Packard's men begin dropping seismic explosives, developed by Randa's seismologist Houston Brooks, to map out the island and prove Brooks' Hollow Earth theory. The unit is then attacked by the giant ape, scattering the survivors across the island. Two groups form between the survivors, one with Conrad, Weaver, the researchers, and one of the soldiers, the other with Randa and the rest. Packard searches for the transport helicopter piloted by Major Jack Chapman, intending to use the weapons on board to kill the ape.

Conrad's group encounters the local Iwi natives and an older Marlow. Marlow tells the group the giant ape, named Kong, protects the island from predators, including a race of subterranean reptilian creatures he dubs "Skullcrawlers" responsible for killing Kong's entire species, leaving him the last of his kind. The Iwi believe when Kong dies, a giant Skullcrawler will awaken and ravage the island. Marlow reveals he and Ikari had become friends during their time on the island, but Ikari was killed by a Skullcrawler some time ago. As Chapman is ambushed and devoured by a Skullcrawler, Conrad's group helps Marlow finish a boat made from parts of Marlow's and Ikari's downed planes. They ride down the river, losing a man to carnivorous birds, and secure communication with Packard's group. When they regroup with Packard, he insists on searching for Chapman.

Marlow leads them through a mass grave of dinosaurs and Kong's family members. The Skullcrawler that killed Chapman attacks them, killing Randa and others before Weaver triggers a flammable gas explosion that kills it. Learning of Chapman's death, Packard reveals his plan to kill Kong and avenge his fallen men. Marlow and Brooks attempt to explain that killing Kong would lead to the Skullcrawlers running rampant, but Packard refuses to listen. The groups part ways, with Packard's group retrieving the weapons from Chapman's chopper and laying a trap for Kong at a nearby lake, while the non-military personnel head back to the boat. Conrad and Weaver meet Kong up-close and, seeing his true peaceful nature, resolve to save him. Packard's group lures Kong with the remaining seismic charges and incapacitates him with ignited napalm. Conrad, Weaver, and Marlow arrive and, after a standoff, persuade the other soldiers to spare Kong, but Packard refuses to yield. As the others retreat, the giant Skullcrawler emerges from the lake, and Kong crushes Packard. The Skullcrawler fights and overpowers Kong, but in the end, the ape is victorious with the humans' help. The survivors reach the rendezvous point and leave the island as Kong stoically watches.

In a pre-end credits scene, Marlow reunites with his wife and meets his son for the first time. In a post-credits scene we see that Monarch detains and recruits Conrad and Weaver. San Lin and Brooks inform them that Kong is not the only monster king and show archive footage of cave paintings depicting Godzilla, Mothra, Rodan, and King Ghidorah. The final image shows Godzilla and Ghidorah in battle.
